Grant Application Checklist
Please review the following guidelines carefully in preparing your application materials.

Maine Arts Commission Application Form submitted electronically
__ Budget Form (if required by program) (Word document)
__ Answers to each of the narrative questions addressed sequentially and by number (Please see guidelines for specific requirements. Please note that both the Master artist and apprentice need to complete narrative sections for the Traditional Arts Apprenticeship program.)
To be mailed separately
__ Artistic and biographical information as required (Please see guidelines for specific requirements)
__ Assurances Page (.pdf document) requires the free Abode Acrobat Reader. (must be printed and mailed to include an original signature.)
All original materials must be postmarked or be hand delivered by 5:00 p.m. on the day of the deadline. Failure to comply with these requirements will result in disqualification of your application. We are not able to accept late applications under any circumstances and only complete applications will be accepted.
